An adjunctive or add-on therapy for major depressive disorder is a medication approved for use in patients who are already taking an antidepressant but may still have unresolved symptoms.

There are many different treatment options. It's never a one-size-fits-all treatment approach. Every patient is different and may respond in their own way to treatment. So working together with your doctor to appropriately manage symptoms is most important.

As a practitioner, we we really have to listen to that individual's story, find out where they've come from, what things they have already tried, how they're doing currently, and if they're still experiencing residual symptoms.

Patients sometimes ask me if adding on to their antidepressant means that their condition is more severe because then they're taking two things instead of one. I really take a step back and try and explain that add-on therapies are because medications have different ways in which they work and some people respond better to one or another.

If you're achieving a partial response to your medication, often your healthcare provider may consider an add-on as an alternative to switching treatments. This may be an appropriate choice for some people because it helps to build on the progress they've already made. So taking add-on therapy to your antidepressant means that you may not have to discontinue the antidepressant medication that you're already on.

There are times it makes sense to add on rather than to switch a medication that’s already provided partial response.

I had a patient who was doing better on an antidepressant medication but was still experiencing unresolved symptoms. He felt like his progress had come to a, to a halt. So he and I worked together and I suggested adding on to his current antidepressant instead of switching antidepressant medications.

At that moment, what popped into my mind is the sign in my office that says, “Don't look back, you're not going that way.” In his case, we felt it made sense to add on to what antidepressant he was already taking rather than switch it. He wanted to continue to move forward.
